The learning disability primary care liaison team is a group of specialist nurses.

They can help you access health services including:

GP

​

​

Dentist

​

​

Optician

​

​

Chiropodist

They can help you to get help for your health problems.

They can attend health appointments with you for support or come to your home.

They can give you information about your health problems and tests that is easy to understand.

They make sure that you have a check of your health every year with the GP to keep you healthy.

They teach group sessions on health topics.

They can also help your carer to support you.
